An anonymous form is one your employees can fill out that doesn't capture any of their personal information. Once a form has been chosen to be anonymous, and is published, it cannot be changed.
To get started go to the Forms Tab, and then go to the manage forms section.
Once on this page, click on the add form button in the top right corner.
From there you'll be taken to the forms Details page.
On this page you'll give the form a name.
Select the form type.
Give the form a description.
Check this box to make it an anonymous form.
When you're ready click the next button, then you'll create the form just like normal.
